Make sure the Personal Narrative is complete (Pages 138-139), including all 3 sections (Gospel Summary, Personal Testimony, and Church Commitment). This can be handwritten in the book or completed in a separate document.

For the Core Commitment (Page 140), please answer each commitment with "I have" or "I will" as appropriate, and sign and date the bottom of the page.
