This work will be covered by an Authorisation to Work and Permit to Work.

By submitting this form, I hereby confirm that the conditions applying to this intended Hot Work have been read and understood fully by all personnel who may be involved with the Hot Work to be carried out, and that the conditions will be adhered to strictly whilst the Hot Work is being undertaken.
By submitting this form, I hereby warrant that the Hot Work detailed herein will be undertaken in accordance with:
(a) the risk assessment and method statement prepared specifically for this purpose by the Hot Work Contractor or vessel’s crew and approved by me; and
(b) the conditions of the Permit to Work issued by me or on my behalf.
On behalf of myself and, as appropriate, the master, owners, and charterers of the vessel, I agree to release and indemnify Peel Ports Group, its servants, and agents from and against all liability for personal injury (whether fatal or not), loss of or damage to property and any other loss, damage, costs and expenses, howsoever caused and which but for the exercise of the permission would not have arisen.
Standard Conditions for Hot Work
1. Hot Work must be undertaken under the charge of and attended constantly by the Responsible Person in Charge or such person appointed by the Responsible Person in Charge for that purpose.
2. Whilst Hot Work takes place, the vessel must be moored securely at the berth specified in the request for permission to undertake Hot Work.
3. Local communication arrangements must be followed whilst Hot Work is undertaken. Details of local arrangements will be provided in writing along with the acceptance confirmation of any consent request.
4. Hot Work must be conducted at such a distance from any material, gas, or vapour as to be incapable (whether directly or indirectly by the emission of sparks or other heated matter) of igniting the same.
5. Hot Work must not be undertaken:
(a) in any tank, hold or other space in which flammable material is present;
(b) in any tank, hold or other space in which flammable liquid has been carried in bulk, or has been carried in containers from which there has been leakage, and which has not been certified by an analyst to be free from flammable vapour;
(c) in any hold or other space in which flammable liquid has been carried in containers from which there has been no leakage but which has not been thoroughly ventilated; or
(d) in the vicinity of any fuel lines, tanks, or vents.
6. Hot Work must not be undertaken whilst bunkering takes place.
7. In the case of the use of oxyacetylene, or other similar types of welding or cutting gear, the gas must be supplied from cylinders that must be placed at such a distance from the Hot Work as not to be affected by heat therefrom.
8. Every precaution must be taken to prevent fire, and the provisions of local Byelaws must be duly observed as applicable.
9. Adequate and ample fire-fighting equipment must be deployed and ready for immediate use, and a fire watch maintained at the location in which the Hot Work is taking place.
10. Adequate provision must be made for quenching and arresting sparks, and for preventing the accidental ignition of material by conducted heat. In particular, if Hot Work is being undertaken in such a position that sparks are liable to be created or fall out of sight of the operator and their assistant, or in such a position that the heat is liable to be conducted to and accidentally ignite material out of sight of the aforesaid, a person must be employed in such a position as to be able to observe and immediately quench such sparks and to prevent such accidental ignition.
11. Where Local Rules require it, Hot Work must be stopped whilst transiting vessels approach and pass.
12. Hot Work must be stopped if any oil or other pollution is observed on the surface of the water in the vicinity of the vessel.
13. Any Hot Work for which Permission has been granted by Peel Ports Group may, at its discretion, be put under the supervision of a person appointed for such purpose by Peel Ports Group. In such cases, Permission to proceed with the Hot Work will be subject to such appointed person being satisfied with the placing, securing and use of Hot Work equipment, and with the precautions taken to prevent the risk of fire.
14. Hot Work equipment must not to be taken to the site or vessel before it is required for use.
15. When taken to or from the vessel, Hot Work equipment must not, in any circumstances, to be passed into a shed, warehouse or any other building.
16. When not in use, Hot Work equipment must be removed from the Dock Estate or stowed within a suitable, secure compartment on board the vessel.
17. A fire watch must be maintained for not less than 30 minutes following the completion of all Hot Work.
18. Peel Ports Group may, at any time, suspend or revoke any Permission granted for undertaking Hot work. Notification of such suspension or revocation of Permission may be communicated by any means determined from time to time by Peel Ports Group.
19. When berthed alongside a cargo operations terminal outside of the operational control of Peel Ports Group, in addition to any other permissions or approvals that are required, the Responsible Person in Charge must obtain the approval of the Terminal Operator before undertaking Hot Work.
