I understand that my child(ren)'s attendance at rehearsals on 11/19, 12/3, 12/10, and 12/17, and at 7pm Lessons & Carols on 12/13 and 4pm Christmas Eve Mass on 12/24 is required for participation in the Christmas Choristers program, and I will make every reasonable effort to ensure they are present.

I understand that, if my child(ren) attend Notre Dame Elementary, they will be directed to the Holy Redeemer Activity Center after school dismissal for rehearsals. If my child(ren) are not Notre Dame Elementary students, I understand that they must be dropped off at the Holy Redeemer Activity Center by 3:00pm for rehearsals.

I understand that I am responsible for picking up my child(ren) at 3:40pm on rehearsal days (parents are invited to observe rehearsals if you wish), and after Lessons & Carols and Christmas Eve Mass.

I also agree to receive email and/or SMS messages from the parish Director of Sacred Music about the Christmas Chorister program and relevant rehearsal/event reminders.

I understand that digital images, photographs, recordings, etc. might be published or shared publicly on social media, newsletters, programs, promotions, etc., and I give permission to St. John Paul II Scioto Catholic Parish to release these images and/or recordings.
